On our previous post, “McAfee Engineers Working On Ebola Anti-virus?” we explored the process a vaccine goes through in order to be released
to the public for treatment. As the post explained, the development of a
vaccine can years to complete. So what other options do Ebola patients have? To this date, more than 15,00 people have contracted the virus and
more than 5,400 people have died. WHO (World Health Organization) has announced two candidate
vaccines are currently in phase 1 trials (clinical trials) and plans are to
bring them to the market as early as January 2015. But, what can be done in the
meantime? Well, there is another way to treat Ebola
patients until a proper vaccine is released to the market. The method is
conducted by transfusing blood, plasma or plasma immunoglobulin fractions from
convalescent donors who have fully recovered from the virus and have developed protective and long-lasting immunoglobulin. In the article, “Ebola virus
convalescent blood products: Where we are now and where we may need to go,” Thierry Burnouf, from the Graduate
Institute of Biomedical Materials and Tissue Engineering, explores the
possibilities of these alternatives.

The Past
The use of human convalescent
blood products first started in the early 20th century. It was
regularly used to treat various infections since vaccines and other ways of
antibiotics were not yet developed. Some of the most common viruses treated by
using this method have been:
· Influenza
· Mumps
· Poliomyelitis
· Typhoid fever
· Herpes zoster
· Measles

The Present
Today, new accomplishments in science and technology allow a more controlled and safer procedures.
Modern blood transfusion is the key factor when preparing convalescent blood products
(See Figure 1).
a.
Whole blood
In this process, a donor needs to be pre-screened before blood can
be collected. Then, blood is collected with an
anticoagulant solution and need to be transfused within 24 hours to maintain clotting
factors. Furthermore, whole blood has to be stored at 2-6°C for up to 35 days.
During a study in 1999, eight Ebola patients were treated with convalescent whole blood; in which seven survived. It should be known that the efficiency of whole blood was not
proved completely during this study.
b.
Plasma
Plasma is obtained by decantation or centrifugation, of whole
blood or by apheresis. Apheresis is the preferred mode of collection for many reasons:
larger volume collected, possibility of more frequent donations, and absence of
impact on the hemoglobin level due to the reinfusion of the red blood cells.
3 The Future
In the absence of a vaccine for Ebola, WHO has approved the use of these alternatives mentioned above under certain conditions that mostly require the safety of patients. At this point, whole blood and plasma seem to be a good alternative to treat patients. However, There are some challenges we need to point out. For example, most countries in West-Africa do not have the proper infrastructures and necessary tools to proceed with this method. Additionally, Having controlled clinical evaluations is also important to establish safe treatments for patients. If regulations established by WHO are followed, this method can be safely applied for future virus outbreaks.

ReplyDeleteEbola has certainly became a major issue once it started to surface. I have a couple questions for you regarding the current status of Ebola. Do you believe that our current containment methods are withholding the virus back? The other question I have for you is how are pharmaceuticals going to fund the production of the anti-virus?
I am a firm believer in regards to having more clinics set up in places that may not be as sanitary or safe from infectious diseases. For example, if more clinics were to be set up in West Africa I feel as though the outbreak could have been much more contained. Here in the US we have only allowed two deaths due to proper measures taken and safety precautions for all citizens. In West Africa, hundreds continue to die as there are no safety measurements set up or even clinics that may be of aid. I feel as though having more clinics can never hurt but only benefit a patient.
ReplyDeleteGreat post. Great presentations. The past, dealing with all the diseases with methods were created for and eradicated for the most part. Something you touched on previously. But now with plasma transfusions not only can we look to eradicate the subjects you discuss, but for people with Hep. C, treatment involves plasma transfusions until certain indicators disappear. Its amazing how we replace dirty with clean through our pipelines in our body.

I've never taken the time to learn the process and it is interesting to know the different methods as well as the stipulations necessary to help ebola patients effectively. I like that this post if very easy to read and simple and clean cut so that it is easy to skim or stay engaged on the topic.

I really enjoyed the format and breakdown that you provided for the disease. I learned a lot of the history of the disease, along with what we know currently, and what should be available in the near future. Is outbreak prevention something that you’re interested in for your future? The picture you included also helped me better understand the process of blood transfusions. Overall, great blog and keep up the good work!
ReplyDeleteI had no idea that convalescent blood products were used before the invention of vaccines and anitbiotics but it definitely makes sense. It is scary to think of the possibility of transmitting a disease from one person to the patient this way though. Also, as you said, many countries do not possess the environment and equipment to perform such procedures that are probably also expensive. I wonder if in this way ebola can be considered a "disease of the poor" since most of those in the news survived from ebola but it is still a huge threat to lives in Liberia and Sierra Leone. I really hope that the vaccines are put on market soon, though even then I don't know if they'll reach all who need them. For the time being I guess this is the best alternative as well as methods of prevention, prevention, and more prevention!

ReplyDeleteThis was a very detailed and informative post. I learned a lot about the possible treatment options for this devastating disease. The options seem promising especially considering the fact that a vaccine is not going to be mass produced at the drop of a hat. I wouldn't trust this vaccine if it did get manufactured so soon and I don't think anybody else should either. Great science takes time and to expect a vaccine now is unrealistic. I really hope these alternatives work because there are thousands of people who are suffering from this disease. Thank you for taking the time to inform people about this topic. Great job!
